【確認事項・Checklist】

  1. １回目の接種は、2024 年 9 月末までに完了してください。

    The first vaccination must have been completed by the end of September 2024.

  2. ２回目、３回目の接種日に関しては、必ず FAQ を確認の上、ご予約をお願いします。

    Please be sure to check the FAQ  before making a reservation about the second and third vaccination dates.

  3. 予診票の住所と住民票の住所が不一致の場合や誤接種時は、全額自己負担になる場合（詳細は FAQ参照）があります。ご了承ください。

    If the address on the preliminary checkup card does not match the address on the resident card, or in case of inoculation date errors, the full amount may be charged to you (see FAQ ).

  4. 予約日にきちんと接種が受けられるよう、対象の方々へ事前に電話連絡をしています。

    迷惑電話設定等をしている方は、下記番号を受信できるようにご準備をお願いします。
